This UPDATE FORM is for SMart Center patients ONLY.
We encourage our patients to update us regularly! Your updates provide us with relevant information in-between visits!
Your written information will be sent directly to a confidential patient email box to be forwarded to the requested treatment professional ONLY.
Please note that this form is for updating purposes only. Any questions you ask will NOT be answered in email format but addressed at your next consultation! See below for additional information on email correspondences.

If you have specific questions and cannot wait until your next appointment you have the following options:
(1) Contact our office to schedule a brief teleconference
* We now offer convenience consultations that are 'last minute' telephone consults scheduled between 8-10 am M-F. These slots are not available for regular appointment scheduling and therefore allow our patients the flexibility to have quick consults on an as-needed basis! NOTE that convenience consults range from 10-30 min and are not meant to take the place of a regularly scheduled appointment.
(2) Contact our office and request an email correspondence.
This is a professional service where patients will be billed based on time. If you request this, our staff will ask you to fill out a brief form that confirms your permission for email corresponding.
(3) Contact our office and request an in-person consultation
This appointment can take the place of your scheduled appointment.
Obviously the ability to schedule an in-person visit is dependent upon availability and YOUR flexiblity.
Our staff can notify you when appointment slots become available.
